The CVR did not record the occurrence flight and several previous flights, which likely due to the inertial switch have been activated during a hard landing when the vertical acceleration exceeded the predetermined value. There was no pilot report of a hard landing therefore the inertial switch did not reset. According to OM-A Section 11.5.14.1, hard landings are included in the list of events to bereported to the company. Therefore, KNKT recommends to SAM Air to ensure any event stated in the list of OM-A Section 11.5.14.1, to be reported in timely manner.

1. QSS Departement issued safety notice number 007/QSS-SN/IV/2023 on 3 May 2023. The safety notice contained recommendation to- Maintenance Manager to ensure the engineer when the aircraft experienced hard landing to check inertial switch CVR remaining ON.- Operation Manager to ensure all the pilot to report immediately to the Operation Manager or Chief pilot when experienced hard landing.2. Operation Departement issued Operation Notice number 05/ON-OPS/V/2023 on 4 May 2023 which requires the pilot make report to the Operation Manager or Chief Pilot when experience Aircraft Handling Difficulties as mentioned in the OM Part A Section 11.5.14.1.
