Isi rekomendasi
The Operation Manual Part A (OM-A) subchapter 8.3.19.3.3 required pilot to conduct approach briefing using Threat and Error management concept.Prior to the approach, the PF briefed to the PM that the approach would be performed as what they usually do. The CVR did not record approach briefing as required by the OM-A.Therefore, the KNKT recommends the Trigana Air Service to ensure pilot conduct approach briefing as required in the OM-A.
