Kronologi & deskripsi kejadian
LNI 145, during climbing cabin altitude was not normal, At FL100 cabin altitude shown around 6000 and keep rising. The Pilot performed manual pressurization and close the outflow valve to get cabin altitude descend. Pilot request to ATC to stopped climb at FL210 but cabin altitude slowly keep rising up to 9000. Cabin Altitude warning sounded and the pilot decided to RTB back to Padang. The pilot performed Emergency descend memory item and NNC as required and after that Oxygen Mask dropped. The aircraft landed safely at PDG and Declared AOG after inspection from engineer. All passengers were fine and the condition was under control

While evaluating the action that had been taken by SIC on the pressurization panel, the PIC selected the outflow valve selector to OPEN position with intention to reduce the cabin rate of climb, subsequently the cabin altitude and cabin rate of climb increased rapidly and the Cabin Altitude Warning sound activated.
